The Sale of Goods Act 1979 makes it an implied term of the contract that goods be as described of satisfactory quality and fit for purpose. As you are in breach of contract I am rejecting the (item) and request that you refund the sum paid to you of (£xxx).
I also require you to confirm whether you will arrange for the (item) to be collected or will reimburse me for the cost of returning it.

I am writing to report a medicine (describe) that I believe to be counterfeit. I saw the medicine for sale on (date) at (describe location or website).
I bought the product but on closer examination I suspected it to be counterfeit because (describe, and give and code or batch numbers or other details from packaging).
I would be grateful if you could investigate this matter for me. (If possible) I enclose a sample of the product and its packaging.
