I am in dispute with the above [bank/ building society] in respect of unauthorised withdrawals made from my current account.

I'm enclosing my complaint form and all the relevant correspondence, from which you will see that I have been unable to reach settlement with [bank & branch], despite having contacted its head office.

I would therefore be grateful if you would consider the case on my behalf.
