25 February – Safety alert: Fake Mounjaro supplied at UK pharmacy

A fake version of Mounjaro has been supplied through one online pharmacy in the UK.
The Mounjaro (tirzepatide) KwikPen 15mg is labelled with batch D873576. The alert applies to solution for injection in pre-filled pen only – Mounjaro KwikPen 7.5mg pens with this batch number are not affected.
Five affected pens, supplied through the Private Pharmacy Clinic in Birmingham, have been identified.
Testing confirmed they do contain tirzepatide, but their manufacturing process is unknown, so sterility cannot be guaranteed. As this is an injectable product, if their contents are not sterile, there is potential for infection and allergic reactions.
The issue was identified due to faults with the pens, where in all but one case the dose knob came off while in use.
The medicines regulator Medicines and Healthcare products Regulatory Agency (MHRA) said the pens are not of the quality and safety standards of legitimate pens and are not safe to use.
It added it has restricted the pharmacy's ability to sell prescription products and investigations are ongoing.
What to do if you have the pen
- Check the batch number on any Mounjaro KwikPen 15mg pre-filled pens you have been dispensed.
- If you have a 15mg pen with batch number D873576, do not use the pen.
- Patients who have this pen should report the case to the MHRA via info@mhra.gov.uk, including 'Mounjaro Pens' in the subject line.
- Patients should keep the pen in a safe place and the MHRA will arrange a collection.
- Anyone who has the defective pen is advised not to try and extract doses from it.
- If you are unsure of how to identify the pen, or are unsure on whether you have administered an affected batch and have any questions, speak to a healthcare professional.
- If you have administered injections using the pen already, the MHRA is reassuring patients any risk to health is low.
- If you have administered injections, and you are experiencing any unusual symptoms that you are concerned about, seek immediate medical attention.
- If you suspect that you’ve had a side effect to these affected pens, or suspect it’s not a genuine product, or have a defective pen you can report it to the MHRA's Yellow Card scheme.

25 February – Safety alert: 2,000 doses of illegal weight loss medicines seized in raids

Almost 2,000 doses of unauthorised weight loss medicines have been sized following raids on two manufacturing plants.
The raids took place as part of an ongoing investigation into an organised criminal network involved in the manufacture and sale of unlicensed weight loss medicines.
Officers from the Criminal Enforcement Unit (CEU) of the Medicines and Healthcare products Regulatory Agency (MHRA) carried out the raids in Lincolnshire and Nottinghamshire.
They found the medicines awaiting dispatch to customers, alongside manufacturing equipment, suspected pharmaceutical ingredients, packaging, and commercial vehicles.
The two premises searched are believed to have been used for the manufacture and distribution of unlicensed weight loss medicines, including retatrutide and tirzepatide, as well as peptide products.
Dr Zubir Ahmed, health innovation and patient safety minister, warned consumers: 'Do not buy weight-loss medicines from unregulated sources. Safe, effective, licensed treatments can make a real difference for those who need them – but they must come from a registered pharmacy, with a valid prescription.'

The Food Standards Agency (FSA) said all other batches of Cow & Gate, Aptamil and Nestle SMA infant or follow-on formula not listed in the product recall notices remain safe to use.
Katie Pettifer, chief executive of the FSA, said: 'Please check if you have Aptamil, Cow & Gate and SMA brands of formula at home. The affected batches are unlikely to still be on shop shelves but may be in your cupboard. If you have an affected batch, do not use it. You can return it to the store and exchange it for an unaffected batch.
'If you are feeding your baby one of the affected batches, stop using it and switch to an unaffected batch from the same brand or another brand.'

She added: 'If your baby’s formula was prescribed, speak to a pharmacist or doctor before changing. The risk of harm to children from cereulide contamination at these levels is low, but consuming it could cause symptoms such as vomiting and diarrhoea. If your baby has recently consumed an affected batch and you are concerned about symptoms, contact your GP or call NHS 111.
'Food businesses have a legal responsibility to sell safe food, including checking ingredients from suppliers are safe. 16 January – More 'unsafe' products added to Hershey Jolly Rancher sweet recall

An alert about several Jolly Rancher sweets that are unsafe to eat has been updated with two more products.
The sweets, manufactured by The Hershey Company, contain Mineral Oil Aromatic Hydrocarbons, (MOAH), which isn’t allowed in food in the UK and may pose a food safety risk if consumed regularly over a sustained period of time.

MG3 owners in the UK are being notified of a recall, three months after it was revealed the car had suffered a critical failure in crash safety tests.

The Driver and Vehicle Standards Agency (DVSA) has confirmed the car is now subject to a recall for the underlying problem to be fixed.
The recall applies to both MG3 and MG3 Hybrid+ models, sold from 2024 onwards.
Which? understands the recall applies to cars manufactured before August 2025.

16 December – Warning for people with allergies not to eat Dubai-style chocolate

People with allergies have been warned not to eat Dubai-style chocolate products after the Food Standards Agency (FSA) found several brands contain products that are not mentioned on the label.
The FSA said some types of the chocolate may be 'unsafe' for people with allergies, because they contain ingredients such as peanut and sesame that are not declared on the label.
The FSA is currently reviewing sampling data from products on sale to check whether they meet the required food safety and labelling standards. Until the work is complete and the full results are known, the FSA is advising consumers with allergies to avoid Dubai-style chocolate as a precaution.
'Dubai-style chocolate has become hugely popular, but we’ve found that some products contain peanut and sesame that aren’t declared on the label. For someone with allergies, this could be dangerous,' said Rebecca Sudworth, director of policy at the FSA.
'With Christmas just around corner, there is a risk that some products on sale may not meet our strict UK standards.
'People with an allergy should not eat Dubai-style chocolate. If you’re buying a gift for someone who lives with allergies, our advice is to avoid buying these products. This includes all allergies, not just peanut and sesame. People without allergies can consume these products, especially where they are supplied by reputable brands and retailers.'

For people who do choose to buy Dubai-style chocolate, FSA advice is to buy from a reputable retailer and check the product label is in English.
The label should contain the following information:
- The name of the food (e.g. milk chocolate with pistachio paste filling)
- A list of ingredients, with allergens emphasised
- The weight of the food in grams
- A best before or use by date
- The name and address of the UK or EU business responsible for the product information. If the food is not from the UK or EU, the name and address of the importer must be included.
There have been several product recalls in 2025 for brands of Dubai chocolate found to contain ingredients not mentioned on the label, including one in November.

10 December – Urgent fire-risk safety check expanded to two more tumble dryer brands

Two more brands have been added to a list of heat pump tumble dryers manufactured by Haier that need an urgent safety repair because of the risk of fire.
Tumble dryers from Caple and Montpellier have been added to a list of models of integrated heat pump tumble dryers manufactured by Haier.
This is in addition to tumble dryers from the brands Baumatic, Candy, Haier, Hoover, Iberna and Lamona.
Owners should check their model number and serial number online to see if their appliance is affected and contact the manufacturer to arrange a repair.
The appliance must not be used until the repair has been completed. If the plug is accessible, owners should switch off and unplug the appliance.
The manufacturer started a corrective action programme in August for 103,000 affected machines.
However the Office for Product Safety and Standards told Haier to halt its initial repair programme because of concerns the modification was still unsafe.
Following an updated modification, Haier has resumed its programme and begun contacting consumers again. Haier initially focused on arranging a second repair for machines that had previously been modified and are now arranging repairs for the remaining 85,000 owners whose appliances still present a fire risk.
For Caple, the affected models have serial numbers: TDi4001 and TDi4000.
For Montpellier, the affected models have a four-digit number on the silver rating label on the rear of the appliance. The affected models have the numbers 2071, 4081, 5251 and 2041.
Should you have any other four-digit number you have no need to call for service. Identification numbers for all the other brands can be found in our original notice, dating from 1 August.
Contact the manufacturer if your model is among those affected. For Caple, you can enter the serial number via their website and for Montpellier you can call 01902 408544.
12 December – 'Serious health risk' warning over FreeStyle Libre glucose monitors following seven deaths worldwide

A safety notice has been issued about certain FreeStyle Libre 3 and FreeStyle Libre 3 Plus sensors which have been found to provide incorrect low glucose readings.
If undetected, incorrect low glucose readings over an extended period may lead to wrong treatment decisions for people living with diabetes, such as excessive carbohydrate intake or skipping or delaying insulin doses.
These decisions may pose serious health risks, including potential injury or death, or other less serious complications.
As of November 14 2025 Abbott Diabetes Care, which manufactures the monitors and issued the notice, has reported 736 serious injuries, and seven deaths globally associated with this issue.
In the US, the Food and Drug Administration (FDA), which regulates medicines and medical devices, said Abbott had issued a letter to distributors, health care providers, and affected customers recommending certain glucose monitor sensors be removed from where they are used or sold.
The affected models are:
- FreeStyle Libre 3 Sensor Model Numbers: 72081-01, 72080-01 Unique Device Identifiers (UDI-DI): 00357599818005, 00357599819002
- FreeStyle Libre 3 Plus Sensor Model Numbers: 78768-01, 78769-01 Unique Device Identifiers (UDI-DI): 00357599844011, 00357599843014
FreeStyle Libre 3 readers and mobile apps are not impacted. Additionally, no other Libre products (FreeStyle Libre 14 day, FreeStyle Libre 2, FreeStyle Libre 2 Plus, or Libre Pro sensors) or Abbott biowearables are impacted.
Patients wearing a FreeStyle Libre 3 sensor or a FreeStyle Libre 3 Plus sensor can find the serial number in the app or reader.
The serial number can also be found on the label on the bottom of the sensor applicator or carton. If patients are using a sensor with a connected insulin delivery device, they can refer to the connected insulin delivery device user manual on how to locate the sensor serial number. There is more information on the MHRA's website.
Abbott is asking owners to confirm if their sensor is affected via www.FreeStyleCheck.com.
Anyone using an affected device should stop using it immediately. Abbott said consumers should use a blood glucose meter or the built-in meter in the FreeStyle Libre 3 reader to make treatment decisions when sensor readings don’t match symptoms or expectations.
Abbott will replace any potentially affected sensors at no charge.

A battery-operated plush walking toy dog has been flagged in an official product safety report for presenting a serious risk of choking and strangulation to young children.
The toy, sold under the Dodowin brand, can break in ways that produce small parts, such as the nose, that fit entirely into a standard small-parts cylinder – a key measure used to assess choking hazards for children under 36 months. There are also no warnings about the attached cord, which could pose a strangulation risk if left unattended.
The toy has been rejected at the UK border.

10 December – Safety alert issued over baby sleep pillows

Parents are being urged not to use products known as 'baby sleep pillows' for babies under 12 months old.
The pillows – products which are marketed or intended to be used as pillows or cushions for babies to sleep on unsupervised – have been linked to child fatalities in the UK and overseas.
The main harms are suffocation and overheating which can lead to serious injury or death in babies. These harms can occur when a baby is placed on the ’Baby Sleep Pillow’ unattended or for sleep including when the pillow is put into a cot or crib.
The Office for Product Safety and Standards (OPSS), which issued the alert, said the pillows are also known by the following terms: Baby Anti-stress Cushion, Baby Comfort Cushion, Baby Cuddle Cushion, Baby Hug Pillow, Baby Pillow, Sleeping Pillow, Stuffed Pillow, Baby Sleep Cushion or Exhaust Pillow.
The OPSS advised consumers not to use the products for any child under 12 months, or to place a baby sleep pillow in a cot or a crib.
It added if the product was marketed for baby sleep, or to be used unattended, you should contact the business that supplied the product as you may be entitled to a refund.
It advised parents and carers to follow NHS safe sleep advice, including placing a baby on its back to sleep and being aware of signs that could indicate overheating, including a baby feeling hot on their tummy, sweating, especially from the face and head, being irritable or breathing faster than usual.

The affected fans were sold by numerous retailers, including Amazon, eBay, Temu, B&Q, Debenhams, The Range and TikTok Shop.

Safety tips for baby and children’s products
Our investigations have uncovered serious risks with some children’s items sold online.
In 2021, we bought 28 toys from AliExpress, Amazon Marketplace, eBay and Wish and found that 12 – or more than 40% of those tested – failed standard safety tests and would be dangerous for children to play with. Some toys failed safety tests multiple times for different hazards.
Check for appropriate safety standards. Any product designed for babies or children can adhere to a specific standard and have a CE/UKCA mark that indicates the product has met EU health, safety and environmental concerns. You may also see the Lion Mark on toys, which indicates toy safety and is used by the British Toy and Hobby Association and Toy Retailers Association.
